地名に由来する名字.渡来人の末裔で近江愛智郡平田郷[滋賀県彦根市稲枝]を本拠とした古代豪族の平田氏がある.延暦6[787]年に平田忌寸枝麻呂が宿禰姓を賜っている.『新撰姓氏録』には平田宿禰は坂上大宿禰と同祖であり,都賀直5世孫の色夫直の後裔としている.また,天安元[857]年には平田忌寸氏と民忌寸氏は伊美吉と姓を改めている.
その他に,中原氏の流れを汲む地下官人の平田氏,大和国の武士団である平田党の一族,桓武平氏梶原氏の末裔で会津蘆名氏四宿老であった平田氏,土佐平田氏,平宗盛の末裔である薩摩平田氏などが知られる.
